Full-page cover with the chart name, owner name, birth details (DOB, time, place, time zone), date of computation, language, and engine version. Practitioner: your logo and contact details.

Every PDF includes — on the inside back cover or in the appendix — the engine version, the ephemeris source (JPL DE441 via Skyfield 1.49), the ayanamsa used (Lahiri unless overridden), and the date of computation.
